Coronary Angioplasty or percutaneous transluminal coronary angioplasty (PTCA). Or percutaneous coronary intervention (PCI) is a kind of a minimally invasive procedure to open the blood vessels that supply blood to the heart muscle via access through groin or hand, unlike bypass surgery there is no need to open the chest. This procedure can also be performed immediately after a heart attack then it is called as primary angioplasty.
Coronary angioplasty is done under local anesthesia. A thin flexible tube called a catheter will be inserted into one of the arteries of the heart through an access from groin or wrist under x-ray guidance called fluoroscopy using special dye called contrast to visualize the blockages on monitor screen.
When the catheter is in place at the origin of coronary artery, a thin wire called coronary wire is maneuvered beyond the length of the affected coronary artery, delivering a small balloon to the affected section of artery. This is then dilated using inflation device to widen the artery, squashing fatty deposits against the artery wall so that blood can flow through it more freely when the deflated balloon is removed.
If a stent is being used, this will be around the new balloon before its inserted. The stent will expand when the balloon is inflated and remains in place when the balloon is deflated and removed. Balloon and wires are removed whereas the dilated stent remains at location of target blockage. Multiple stent can be put in multiple vessel as needed. A coronary angioplasty usually takes between 30 minutes and 2hrs hours. After coronary angioplasty the patient is observed in ICU for 1 to 2 days after which they are discharged with medicines as per clinical requirement. Patient is called for regular follow-up after discharge.
